While yard hostlers are moving an Allegheny and West Virginia Railroad (AWVR) train, pulled by lead locomotive #777 at a trainyard in northern Pennsylvania, Dewey, the engineer of the mixed-freight train, realizes that a switch is not correctly aligned and tries to realign it by leaving the cabin of the moving locomotive after setting the throttle to idle, but he trips and falls, leaving the locomotive unattended going down the main line.
